Portola Valley Insurance Tracking Form
This form tracks changes in PV home insurance such as home insurance cancellations, home insurance premium changes, and home insurance coverage changes within Portola Valley in the last 3 years. You may also provide the town with information on your existing insurance policy or on your lack of insurance.
A PV resident may take this survey multiple times during any given year. For example, if you had a home insurance premium increase you would fill out this form. If you decided to change insurance companies later that month, you could fill out the form again. If your policy was not renewed or cancelled later that year, you could fill out the form a third time.
Have your insurance ready to answer questions on rate increase and date of policy changes.

This form will not capture your email information. The data collected will only be shared with the Town staff, the Town Council, and State officials. You do not need a Google account to fill out this form. If you are logged into Google when you open this form, your account name will be displayed, and you have the option to save your progress during data entry.
